Bollinger Insurance Certificate Request Form Instructions:

The ULA no longer handles Field Insurance Request Forms.

First and foremost, Bollinger is no longer verifying rosters. The procedure is now for the individual teams to verify their own rosters using the US Lacrosse member look-up feature found at https://secure.uslacrosse.org/m.cfm. After they have verified that all their players are US Lacrosse members, then they can continue on our website to create & issue their own Certificates.

To create Certificates, they would head to BollingerLax.com and select the “Certificate Request” menu option from the homepage. Since they are 100% US Lacrosse registered, then they would be selecting “Certificates for member teams and leagues” from the following screen. The next screen will ask if they have a user name & password. To obtain a user name & password, they can email LacrosseInfo@BollingerInsurance.com. A representative will get back to them immediately if they email between the hours of 8am – 4pm Eastern Standard time Monday-Friday.

Once they enter the user name and password, the following screen is extremely important. This instructional page explains who should be issued a Certificate, how to issue a Certificate and so on. It also explains that if a liability claim is filed, the team administrator must produce a final roster of all participants which will be verified for current US Lacrosse membership from the date of loss. The team administrator must also certify that they have verified all their participants (including coaches) US Lacrosse membership status’ in order to continue on to create their Certificates.

This is a great new development for US Lacrosse and Bollinger. Although the responsibility for team administrators to take an active role in maintaining US Lacrosse membership and verifying their participants has increased, the availability of insurance Certificates has also increased. You can issue a Certificate whenever you need it the most – even on the weekends.
